Carer's medical report (CARMed1)

Carer's Allowance, Carer's Benefit and the Carer's Support Grant share one medical report, completed by the doctor of the person who needs care.

The form itself is Department of Social Protection’s: the official form, on Department of Social Protection’s site (link checked 24 October 2026). Below is what each person fills in, where it goes, and the document we provide beside it.

What it asks for

Completed by the doctor of the person being cared forThe consultant completes

  • The person's details
  • Level of care and attention required
  • Diagnosis and duration of the condition
  • Doctor's signature, IMC number and stamp

Where it goes

  • The report assesses the level of care and attention required and is completed by the doctor of the person being cared for when the carer applies for Carer's Allowance, Carer's Benefit or the stand-alone Carer's Support Grant.
  • For confidentiality, the report may be sent to the department's Chief Medical Advisor rather than with the application.
  • The department's guidelines say the medical certificate incorporated in the CR1 form must be completed and signed by a doctor attending the person.

Using it well

  • Check whose doctor is being asked: the report belongs to the person receiving care, who may not be your patient's relative on the file.
  • Offer the Chief Medical Advisor route when the diagnosis is sensitive.

Questions

Who completes the Carer's Medical Report?

The doctor of the person being cared for, not the carer's doctor, when the carer applies for Carer's Allowance, Carer's Benefit or the Carer's Support Grant.

Can the medical report be sent separately for confidentiality?

Yes. The form says that, for reasons of medical confidentiality, the report may be sent to the department's Chief Medical Advisor.

Is a report needed for a child on Domiciliary Care Allowance?

The department's Carer's Allowance page says no medical report is needed when caring for a child getting Domiciliary Care Allowance.

In Ask Brigid this paperwork sits beside the diary and the record: The patient record in Ask Brigid.
