Disability Allowance medical report (DAMed1)

The DA1 application carries a medical report, DAMed1, that the patient's doctor completes and that should travel with any specialist reports the practice holds.

The form itself is Department of Social Protection’s: the official form, on Department of Social Protection’s site (link checked 24 October 2026). Below is what each person fills in, where it goes, and the document we provide beside it.

Where it goes

  • The patient signs the permission to release medical information, then the doctor completes the Medical Report (DAMed1).
  • The doctor is asked to enclose copies of recent specialist reports and any test results so the department has a full picture when deciding the claim.
  • A special fee is paid for completing and returning the report; the doctor's DSP panel number goes in the box provided to ensure payment.

Using it well

  • Pull the specialist reports and results from the chart before the consultant sits down; the form asks for them as enclosures.
  • Record the DSP panel number once in practice settings so it is to hand for every social-welfare form.

Open it in the sample practice

The sample practice is the real Ask Brigid on invented patients, open without a sign-up. This document joins its Library with the next sample-practice update; the Library already holds the practice’s templates.
