Irish Life Health consultant registration
Irish Life Health pays registered consultants only; registration is a form, a CV and your hospital contract, by email.
The form itself is Irish Life Health’s: the official form, on Irish Life Health’s site (link checked 24 October 2026). Below is what each person fills in, where it goes, and the document we provide beside it.

What it asks for
What to sendThe consultant completes
- Completed consultant registration form
- Summarised copy of your most recent curriculum vitae
- Copy of your current hospital contract as a consultant in Ireland
Where it goes
- Complete the registration form and send it with a summarised CV and a copy of the current hospital contract to partnersupport@irishlifehealth.ie.
- The general ground rules say the consultant must be registered with Irish Life Health before the relevant procedure is performed.
- Help with registration: 01 562 5162, Monday to Friday 9am to 3pm.
Using it well
- Register before the first Irish Life Health patient is listed; the ground rules tie payment to prior registration.
- Keep the registration confirmation with the practice's insurer records.
Open it in the sample practice
The sample practice is the real Ask Brigid on invented patients, open without a sign-up. This document joins its Library with the next sample-practice update; the Library already holds the practice’s templates.
