Vhi Healthcare
Vhi consultant registration form
Before Vhi pays for your work, you register as a provider; the form comes with a document checklist that decides how long it takes.
The form itself is Vhi Healthcare’s: the official form, on Vhi Healthcare’s site (link checked 24 October 2026). Below is what each person fills in, where it goes, and the document we provide beside it.

What it asks for
Documents for all applicantsThe consultant completes
- Full comprehensive curriculum vitae
- Evidence of Specialist Division registration (Medical Council of Ireland or GMC)
- Colour copy of passport with photograph and signature
- A clear recent photo
- Revenue Commissioners documentation confirming tax registration name, number and type
Where it appliesThe consultant completes
- HSE external work form, signed, for off-site private practice
- Letter of practising privileges from each private hospital
- Group Service Provider Agreement appendix or precedent partner letter
Practice contactsThe secretary completes
- Practice manager or secretary details for eSchedule access
Where it goes
- Details go to provider.networks@vhi.ie; registration is effective from the date Vhi approves it.
- A consultant holding a Type A public hospital contract is not eligible and does not complete the form.
- Approval must be sought from Vhi to add a private facility to a registration before services are provided there.
Using it well
- Gather the Revenue letter first; Vhi accepts only official Revenue Commissioners documentation for the tax details.
- Explain any gaps in the CV in the covering note, as the form asks.
Open it in the sample practice
The sample practice is the real Ask Brigid on invented patients, open without a sign-up. This document joins its Library with the next sample-practice update; the Library already holds the practice’s templates.

Questions
What does Vhi ask for with the consultant registration form?
A full CV, evidence of Specialist Division registration with the Medical Council of Ireland or the GMC, a colour passport copy, a recent photo, and official Revenue documentation of the tax registration name, number and type. Private-hospital applicants add letters of practising privileges.
Why does Vhi want the secretary's details?
So the practice manager or secretary can register for the eSchedule. Their email and mobile number cannot be the same as the consultant's.
When does the registration take effect?
From the date of approval by Vhi, not from the date of application.
In Ask Brigid this paperwork sits beside the diary and the record: Ask Brigid for consultants.
