Irish Life Health
Irish Life Health pre-authorisation request
Certain procedures are only covered when Irish Life Health approves them in advance; the health care provider asks in writing and hears back within 15 working days.
The form itself is Irish Life Health’s: the official form, on Irish Life Health’s site (link checked 24 October 2026). Below is what each person fills in, where it goes, and the document we provide beside it.

Where it goes
- To apply for pre-authorisation, the health care provider submits a request in writing to Irish Life Health, which assesses it within 15 working days.
- All PET-CT scans must be pre-authorised and need a consultant referral; the request form is completed by the requesting consultant.
- Overseas pre-approval: part 2 of the form is completed by the GP or consultant, and the pre-authorisation ends after six months or at the end of the policy year, whichever is sooner.
Using it well
- Attach the history the PET-CT form asks for rather than summarising it on the form.
- Diary 15 working days from sending; chase on day 16, not before.
Insurer claim submission is not in Ask Brigid’s launch release.
Open it in the sample practice
The sample practice is the real Ask Brigid on invented patients, open without a sign-up. This document joins its Library with the next sample-practice update; the Library already holds the practice’s templates.

Questions
How long does Irish Life Health take to decide a pre-authorisation request?
The handbook says the request is assessed as soon as possible and in any case within 15 working days.
Is there a form for PET-CT pre-authorisation?
Yes: the Request for Pre Authorisation for a PETCT or a PSMA scan, completed by the requesting consultant with the indications, history and the reason PET-CT is chosen over other techniques.
Does Ask Brigid send the request?
No. The companion letter is a request template the practice fills and sends itself; insurer claim submission is not in the launch release.
In Ask Brigid this paperwork sits beside the diary and the record: Letters in your house style.
