Laya Healthcare
Laya Healthcare pre-authorisation request
Where Laya's Schedule of Benefits says a procedure needs pre-authorisation, the consultant asks for it in writing before the admission.
The form itself is Laya Healthcare’s: the official form, on Laya Healthcare’s site (link checked 24 October 2026). Below is what each person fills in, where it goes, and the document we provide beside it.

What it asks for
The request letterThe consultant completes
- Member name and Laya membership number
- Procedure and procedure code
- Clinical indication
- Proposed hospital
- Proposed date of treatment
- Consultant name and Laya provider details
Where it goes
- If a treatment or procedure needs pre-authorisation, the Schedule of Benefits says so, and the consultant submits a request in writing to Laya.
- Medical tourism, treatment not available in Ireland and gender affirmation surgery need a fully completed pre-approval form from the Irish-based referring consultant, who must be fully participating with Laya, and written approval before travel.
Using it well
- Send the request before the admission is booked; the hospital will want Laya's written approval on file.
- Keep the approval with the admission paperwork and note any expiry Laya states on it.
Insurer claim submission is not in Ask Brigid’s launch release.
Open it in the sample practice
The sample practice is the real Ask Brigid on invented patients, open without a sign-up. This document joins its Library with the next sample-practice update; the Library already holds the practice’s templates.

Questions
Who sends the pre-authorisation request to Laya?
The consultant, in writing. Laya's rules say the Schedule of Benefits, which the consultant has access to, states which procedures need it.
Is there a Laya pre-authorisation form for Irish treatment?
Laya's general rules describe a written request from the consultant rather than a public form; a pre-approval application form is named for treatment abroad and gender affirmation surgery.
Does Ask Brigid send the request?
No. The companion letter is a request template the practice fills and sends itself; insurer claim submission is not in the launch release.
In Ask Brigid this paperwork sits beside the diary and the record: Letters in your house style.
