Laya settles most hospital stays directly: the patient signs a form on arrival and on leaving, and the hospital deals with Laya.
The form itself is Laya Healthcare’s: the official form, on Laya Healthcare’s site (link checked 24 October 2026). Below is what each person fills in, where it goes, and the document we provide beside it.

Where it goes
- Laya has direct settlement with almost all hospitals and consultants: the member fills out a form on arrival, signs it on leaving, and the hospital deals directly with Laya.
- Before going in, the member checks cover with the hospital, consultant and procedure code to hand, on 021 202 2000.
- Out-patient receipts must reach Laya within twelve months from the end of the policy year, or the benefit is not paid.
Using it well
- Give the patient the procedure code in writing before they ring Laya; the cover check cannot be done without it.
- Keep the signed hospital form reference with the admission in the patient record.
Insurer claim submission is not in Ask Brigid’s launch release.

Is there a Laya hospital claim form the consultant fills in?
For most stays the claim is settled directly between Laya and the hospital; the patient signs a claim form in hospital and the hospital deals with Laya. Laya's page says you do not usually need to submit an in-patient claim form yourself.
What does the secretary need before the patient is admitted?
The hospital, the consultant's name and the procedure code, so the patient can confirm cover with Laya before the admission.
Does Ask Brigid submit the claim to Laya?
No. Insurer claim submission is not in the launch release. The companion sheet records the procedure code, the hospital and the membership number for the admission.
