Level Health
Level Health pre-authorisation request
Level Health takes pre-authorisation requests through its provider portal or by email, and PET-CT scans need their own form.
The form itself is Level Health’s: the official form, on Level Health’s site (link checked 24 October 2026). Below is what each person fills in, where it goes, and the document we provide beside it.

What it asks for
The requestThe consultant completes
- Member name and Level Health membership number
- Procedure and procedure code
- Clinical indication
- Proposed hospital
- Proposed date of treatment
- Consultant name and Level Health doctor code
Where it goes
- Pre-authorisations go through the online portal or by email to preauthorisation@levelhealth.ie.
- PET-CT scans are subject to pre-authorisation by Level Health and have their own form, submitted to preauthorisation@levelhealth.ie.
Using it well
- Use the portal where the practice has a login; the email route is the fallback the provider page names.
- Keep the approval reference with the admission paperwork.
Insurer claim submission is not in Ask Brigid’s launch release.
Open it in the sample practice
The sample practice is the real Ask Brigid on invented patients, open without a sign-up. This document joins its Library with the next sample-practice update; the Library already holds the practice’s templates.

Questions
How does a consultant request pre-authorisation from Level Health?
Through the online provider portal, or by email to preauthorisation@levelhealth.ie, according to Level Health's provider page.
Is there a separate form for PET-CT?
Yes. Level Health's PET-CT pre-authorisation form says PET-CT scans are subject to pre-authorisation and the request goes to preauthorisation@levelhealth.ie.
Does Ask Brigid send the request?
No. The companion letter is a request template the practice fills and sends itself; insurer claim submission is not in the launch release.
In Ask Brigid this paperwork sits beside the diary and the record: Letters in your house style.
