Injuries Resolution Board (PIAB)
PIAB medical assessment form (Form B)
Every personal-injury claim to the Injuries Resolution Board starts with a medical report from the claimant's treating doctor, on Form B.
The form itself is Injuries Resolution Board (PIAB)’s: the official form, on Injuries Resolution Board (PIAB)’s site (link checked 24 October 2026). Below is what each person fills in, where it goes, and the document we provide beside it.

What it asks for
The reportThe consultant completes
- Injuries suffered and their nature
- Relevant medical history only
- Treatment given and prognosis
- Neck Disability Index, where appended
Practice sideThe secretary completes
- Fee charged to the patient
- Date the form was requested and returned
Where it goes
- To initiate a claim, the Board requires the claimant to ask their treating doctor to complete the Medical Assessment Form; no new examination is required and it can be completed from the medical records.
- A copy of the report goes to the respondents and their insurers, so it should contain only medical history relevant to the claim.
- The patient pays for completing the form; reasonable charges form part of the Board's assessment and the patient seeks to recoup them.
Using it well
- Invoice the patient for the report before it leaves the practice; the Board's process assumes the claimant pays and recoups.
- Write from the chart. The Board does not ask for a fresh examination.

Questions
Do I need to examine the patient again to complete Form B?
No. The Board says no new medical examination is required and the report can be completed from the medical records.
Who pays for the PIAB medical report?
The patient. The Board says reasonable charges form part of its assessment of the claim and the patient seeks to recoup the cost through the final assessment.
What should the report leave out?
Anything not relevant to the claim: the report is passed to the respondents and their insurers, so it should only contain medical history relevant to the claim being made.
In Ask Brigid this paperwork sits beside the diary and the record: Reports and letters in your house style.
