NDLS driving licence medical report form

Drivers over 75, bus and truck applicants and anyone with a listed condition need a medical report completed in person by a registered medical practitioner.

The form itself is National Driver Licence Service’s: the official form, on National Driver Licence Service’s site (link checked 24 October 2026). Below is what each person fills in, where it goes, and the document we provide beside it.

Where it goes

  • The driver must be present in person for the consultation; the form is signed by the applicant in the presence of the certifying registered medical practitioner.
  • The report must be presented to the NDLS within one month of being signed by the medical practitioner.
  • Where a report is required, a separate eyesight report is not needed unless the doctor indicates it.

Using it well

  • Book these as in-person visits; the NDLS will not accept a form from a video consultation.
  • Date the signature and remind the patient of the one-month window when they leave.

Open it in the sample practice

The sample practice is the real Ask Brigid on invented patients, open without a sign-up. This document joins its Library with the next sample-practice update; the Library already holds the practice’s templates.
