- Admitted, discharged, consultant and ward
- Reason for admission
- Diagnosis
- Clinical course and treatment
- Medication changes and discharge medicines
- Allergies and adverse reactions
- Follow-up: owner, actions and advice
- Sign-off: name, qualifications and Irish Medical Council number
Using it well
- List stopped and changed medicines as well as new ones.
- Name the owner of each follow-up action, so nothing falls between the hospital and the GP.
