What is on the form
- Practice letterhead, date and reference
- Patient: name, date of birth, MRN and address
- GP, their practice, and anyone copied in
- Reason for review
- Assessment and investigations
- Diagnosis
- Plan and recommendations
- Sign-off: name, qualifications and Irish Medical Council number
Using it well
- Lead with the answer the GP needs: the diagnosis and the plan, then the detail.
- Say who owns each next step: the GP, the patient or the practice.
- Copy the patient where your practice does so; the cc line is there for it.
