The in-patient and day-case claim form Vhi settles directly with the hospital; the admitting consultant owns six of its eleven sections.

Where it goes
- Where Vhi operates a direct payment arrangement it pays the hospital benefit direct to the hospital or treatment centre and sends the member a statement of the benefits paid.
- Section 1 is completed by the hospital's administration staff; sections 6 to 11 by the admitting consultant.
- Postal claims go to Vhi, PO Box 11530, Dublin 18.
Using it well
- Have the consultant code, the clinical indicator code and the procedure dates to hand before the consultant sits down with the form.
- Keep a copy of the completed consultant section in the patient record, filed with the admission.
