What is on the form
- Practice letterhead, date and reference
- Patient: name, date of birth, MRN and address
- Visit date, specialty and subject
- Presenting complaint
- Assessment
- Plan
- Follow-up
- Sign-off: name, qualifications and Irish Medical Council number
Using it well
- Write it for the reader it goes to: plainer language for the patient, more detail for the GP.
- Put the follow-up date and who books it in the last section.
